Job Description

The successful candidate will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing education to patients and their families. 

Job Duties:

  • Provide patient education on health maintenance and disease prevention.
  • Assess patient's physical, psychological, and social status.
  • Develop and implement a plan of care in collaboration with the patient and family.
  • Monitor the patient's progress and adjust the plan of care as needed.
  • Administer medications, treatments, and therapies as prescribed by the physician.
  • Document all patient care activities in accordance with agency policies and procedures.
