Home Health Registered Nurse (Full Time interest only)
We are seeking a compassionate, dependable, and experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes for one of our clients. This full-time position is ideal for an RN who values building lasting relationships with patients while delivering high-quality, individualized care. The RN will work collaboratively with physicians, caregivers, and the interdisciplinary care team to promote positive patient outcomes and reduce hospitalizations.
Essential Responsibilities
Perform comprehensive nursing assessments and develop individualized care plans.
Administer medications, treatments, and wound care as ordered by the physician.
Monitor and document patient condition, progress, and response to treatment.
Educate patients and family members on disease management, medications, safety, and self-care.
Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
Identify changes in patient condition and communicate appropriately with the healthcare team.
Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
Participate in case conferences and interdisciplinary team meetings.
Ensure compliance with all state, federal, and agency guidelines.
